Hilary Thompson, independent chair of the Birmingham Safeguarding Children Board, said: “Following a meeting of relevant professionals, and on advice, I have decided that a serious case review will be conducted into the death on Sunday of a two-year-old boy in the Ward End area of Birmingham. It will be conducted under statutory guidelines and published in due course.”
